The role of the Interdisciplinary Professional (IDP) clinician is to support the South Australian Spinal Cord Injury Service (SASCIS) rehabilitation team in the coordination of rehabilitation processes, referrals and discharge plans for SASCIS clients on the subacute rehabilitation unit.
Working collaboratively with the multidisciplinary SASCIS team, patients and their families/carers, the IDP clinician will help ensure family meetings, goal documents and ongoing discharge plans are efficiently delivered by the team.
The incumbent will undertake interdisciplinary screening of patients to assist in identifying broad health or social impacts and issues for goal setting and discharge planning.
As SASCIS service needs change, the IDP clinician may work across the patient pathway within SASCIS, including acute, subacute and ambulatory settings. They may also be required to work across CALHN to meet organisational demands.
Working in a state wide specialist service, the IDP clinician will have specialist knowledge in the healthcare needs and multidisciplinary management of people with spinal cord injury and similar complex neurological impairments.
The IDP clinician will have a co-ordination role for NDIS eligible patients, throughout the inpatient journey
The IDP clinician will coordinate and lead on development for an effective patient education program and a multidisciplinary team education program in specialist spinal rehabilitation topics.
