What you'll be doing

The Nurse Practitioner (NP), Aged Care will function autonomously and collaboratively in an advanced and extended clinical role to provide safe, high quality advanced nursing care. The NP Aged Care will primarily perform nursing assessment, management, treatment and clinical support to older persons. The NP will contribute to the advancement of quality health care systems within the Local Health District and across Residential Aged Care Facilities (RACF’s) to reduce avoidable hospital presentations and promote early discharge from hospital.

The core aspects of the role will be the delivery of safe, quality practice including the development of specialist plans of care that meet the needs of the older person in collaboration with the medical team. The NP will also have a collaborative link with Hospital in the Home services on the Central Coast.

The NP role includes the assessment and management of older people using nursing knowledge and skills and may include but is not limited to initiation of diagnostic investigations, prescribing of medications and direct referral of patients to other health care professionals. The NP will practice within the NP’s approved scope of practice (ScOP), competence, legislative and policy frameworks and the strategic direction of the service and CCLHD.

The NP will:

Provide advanced clinical nursing care/interventions as authorised by the NSW Nurses and Midwives Registration Board.
Develop and have an approved ScOP that is in line with the NMBA and Nurse Practitioner Policy identifying context and an evaluation strategy
Develop and monitor measurable Nurse Practitioner practice outcome indicators and continually review own ScOP
Ensure a patient centred advanced nursing practice in the assessment of the older person and implement management strategies in liaison with the medical officer/team, nursing and allied health staff
Facilitate processes to ensure involvement of family and carers in assessment and management strategies and provide information and support for their ongoing needs following discharge of patients from acute care settings.
Provide education on clinical issues to patients and carers within the clinical environment.
Establish communication links and work collaboratively with RACFs to support/promote management of aged care patients.
Establish interactive and networking relationships with General Practitioners responsible for the ongoing care of clients.
Contribute to the Hospital in the Home strategy.
Contribute to hospital avoidance and improvement of the patient journey through early intervention and patient-centred care planning.
