Creating Links is a growing not-for-profit community organization positioned in Bankstown, Camden and Faulconbridge, that focuses on developing dynamic, resilient, confident and inclusive communities and families.

About the Role:

We are seeking experienced, passionate, vibrant and committed Support Workers to join our Team within our TACP Program, which provides short-term restorative care to optimise the functioning and independence of older people, after a hospital stay. This program is goal-oriented, time limited and therapy focused.

Our clients are based in Bankstown, Camden, Faulconbridge and surrounding suburbs.

As a TACP Support Worker, your duties will include:

Delivering high-quality person-centred support to people over 50yo;
Providing a high level of personal care including showering, medication prompting and wound care;
Domestic assistance, such as cleaning, mopping or changing bed linen;
Social support, including shopping and transporting the participants to scheduled appointments;
Other duties as required.

The ideal candidate will meet the following criteria:

Completed Certificate III in Individual Support (or equivalent);
Minimum 1-year paid-working experience in Aged Care or TACP;
Experience in an AIN role;
Good verbal and written communication skills;
Ability to work flexible hours from Monday to Sunday;
Class C Driver’s Licence and access to a vehicle with comprehensive car insurance;
Current First Aid Certificate;
Valid Working with Children Check Clearance and National Criminal Records History Check;
Vaccination records.
