The Nurse Practitioner's role within the multidisciplinary team is to increase access to high quality and cost effective care, streamline the patient journey between acute and primary health care services and enhance health outcomes for individuals, families and communities. This position focuses on early detection and effective management of clients with or at risk of diabetes in remote CA communities. The position provides an outreach service requiring travel to remote communities for up to week and therefore requires a manual driver's licence.

The successful applicant will have a recognised vaccination provider qualification, have current registration with Nursing and Midwifery Board of Australian and hold a graduate Diploma in relevant clinical Speciality.
