What you'll be doing

Provide specialist Social Work service to patients, clients, consumers and their family/carers to ensure the provision of optimal health outcomes consistent with ISLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct. This position provides the above Social Work service to the Shoalhaven Transitional Aged Care Service (STACS). STACS provides a time-limited, goal orientated service for older people who would benefit from additional low intensity therapy in their own homes as well as assisting with the decision making process around long term care

The social worker will also maintain social work and case management responsibility for a number of cases until discharge. Duties include early psycho-social assessments, care planning, education, advocacy for the needs of clients, families and/ or carers, liaise with community agencies, the client's General Practitioner, counselling and carer support.

The position functions as part of an inter-disciplinary team with a teamwork approach to the provision of low intensity therapy home-based programs to frail elderly clients who are ready for discharge from sub-acute hospital care, but are unable to return home without additional care and support. There is a strong working relationship with the Illawarra Shoalhaven Aged Care Assessment Team (ACAT) and with a Site Social Work Unit Head for professional supervision.

Selection Criteria

Social Work degree and eligibility for the Australian Association of Social Workers (AASW), including the ability to demonstrate a practical application of the AASW Code of Ethics.
Extensive post graduate experience in the provision of Social Work services in a health related setting
Demonstrated skills in case management with high level assessment and intervention skills and the ability to manage own caseload through co-ordinated assessment and care management of older people with complex needs.
Extensive knowledge of issues related to working with older people and knowledge of Commonwealth Government guidelines on aged care residential assessments, the Aged Care Act (1997) and Transitional Aged Care Programs.
Demonstrated high level ability to communicate clearly and succinctly both verbally and in writing, and proven ability to work effectively and collaboratively within a multi-disciplinary team. Demonstrated competence in the use of electronic information systems
Demonstrated ability to prioritise work and manage stressful situations; ability to work unsupervised and apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel or critical nature .
Demonstrated ability to initiate, lead, complete and share quality improvement activities and service evaluation processes with other staff and demonstrated capacity to provide clinical supervision.
Current NSW Drivers Licence, with the ability to travel in accordance with position responsibilities.
