Arcare is an approved provider with the Department of Health and provides Government Funded Home Care Packages, Private Aged Care and NDIS Services across Queensland and Victoria. We are looking for a special person to join our team coordinating client care and support services across the Greater Brisbane Region.

To be successful in this Care Consultant (Clinical) role you will be a Registered Nurse with a passion for working in the Community. You will bring excellent communication, assessment, care planning, monitoring and customer service skills.

This dual role involves management of an allocated case load of Home Care Package clients, as well as the provision of high-quality clinical assessment, advice and support around client care and undertakes client clinical assessment compliant with relevant legislation, guidelines, practices, policies, and procedures.

You will be responsible for ensuring that our clients are supported to live independently in their home through provision of Consumer Directed Care. You be will skilled at goal planning, referral for appropriate services, ongoing monitoring and review, within both the Home Care Package Operational Guidelines and their allocated Home Care Package budget.
A mobile phone and laptop will be provided for your use. A shared Home Care Pool Vehicle is available for use during work hours. Use of your own vehicle may also be required, you will be paid for your work-related km’s travelled.

To be considered for this rewarding role you will have:

Tertiary Qualification in Nursing
Hold and maintain current Nursing registration with the Nursing and Midwifery Board of Australia
Current AHPRA membership registration
Essential to have previous experience working in the aged care and or the disability field, ideally as a Care Manager / Case Manager
Current National Police Clearance (within 6 months of check), Working with Children’s -Blue Card and Disability Services Positive Exemption Notice Card (Yellow Card) or willingness to obtain
A valid Driver’s Licence and reliable vehicle
Previous experience in Home Care Packages or relevant programs is required
Demonstrated experience in managing client budgets
Experienced in assessment and record keeping via databases and or client management systems. Carelink + experience would be advantageous
Able to work autonomously and as a part of a team
Able to manage conflicting priorities – excellent time management skills required
Ability to work within Organisational Policies and Procedures and Legislative Requirements
Computer literacy with Microsoft Office (Word, Excel, Outlook) and the Internet is required
Willing to Use Own Vehicle when Required (km paid)
All successful applicants are encouraged to have an influenza vaccination as an inherent requirement of the role
All applicants are required to undergo a National Police Check and provide evidence of right to work in Australia
